Members also receive a 15% discount on all other dental treatment, including:
- Fillings
- Crowns
- Root Canals
- Orthodontics

- Additional dental cleanings

Individuals with insurance are not eligible for membership
Membership is non-transferable and cannot be split with any other patient
Membership runs for 365 days from the date of purchase
No refund offered if services are not used within 365 days
This is not an insurance plan
Membership is only valid at Prescott Family Dental of Prescott, WI
Cannot be combined with any other discount, such as cash or senior discount
No discounts rendered at specialty dental clinics. Referrals to specialty clinics are at the doctors’ discretion
